By accessing or using the Decades by Longevity Health website (the "Site") or any of the services made available through it (the "Services"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, you should not access or use the Site or the Services.
You must be at least 18 years old to use the Site or request Services. Medical services are available only to individuals physically located in states where the applicable Longevity Health affiliated medical practice and a licensed provider are authorized to provide care. Eligibility for treatment is determined by the treating provider in their sole clinical judgment.
The Site is intended to provide information about Decades by Longevity Health and our Services, and to allow eligible users to complete an eligibility questionnaire and request care. You agree to use the Site only for lawful purposes and to provide accurate, current, and complete information when submitting any questionnaire or request.
Visiting the Site, completing an intake questionnaire, creating an account, or submitting payment does not establish a physician–patient relationship. A physician–patient relationship is established only after a licensed provider affiliated with the applicable Longevity Health affiliated medical practice has reviewed your information and affirmatively agreed to provide care.
Approved patients receive care via telehealth through the applicable Longevity Health affiliated medical practice authorized to provide services in the patient's state. Your use of telehealth Services is also governed by our Telehealth Informed Consent, which is incorporated into these Terms by reference.
Certain Services, including physician review, require payment before review takes place. Medication costs are billed and paid directly to the pharmacy you select; we do not bill insurance and do not collect medication payments on the pharmacy's behalf. If, after review, a physician determines that treatment is not clinically appropriate for you, refunds will be handled in accordance with our program policies as communicated to you at the time of purchase.
Your information is handled in accordance with our Privacy Policy, which describes how we collect, use, disclose, and safeguard personal and health information. By using the Site or Services, you acknowledge that you have reviewed the Privacy Policy.
All content on the Site, including text, graphics, logos, branding, images, design elements, and other materials, is owned by or licensed to Decades by Longevity Health and is protected by applicable intellectual property laws. You may not copy, reproduce, modify, distribute, or create derivative works from any Site content without our prior written permission.
Information made available on the Site is provided for general informational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Nothing on the Site should be relied upon as medical advice outside of a properly established physician–patient relationship.

Decades by Longevity Health may update these Terms from time to time. When we do, we will revise the "Last updated" date above. Your continued use of the Site or Services after any revision becomes effective constitutes your acceptance of the updated Terms.
